Description

Providing wound prevention and management in Post-Acute/Long-Term Care settings is different from other sites such as out-patient and acute care. In addition, interventions for prevention and management of chronic wounds is very challenging for many reasons in LTC including, but not limited to; the population (older and/or more fragile), the regulations, reimbursement trials, and clinical knowledge challenges by direct care staff. All of these and more issues can make wound management in LTC demanding for building management, leadership and clinical and support staff. This program will integrate clinical best practices with regulatory (survey) and reporting (MDS, Section-M) mandates for wound prevention and care, providing the attendees with up-to-date information to assist clinical teams and building management and leadership with tools and direction for creating a functional wound prevention and care program. This back-to-basics wound prevention and management program is for ALL staff; hands-on care and leadership staff, including components that are appropriate for CNAs, DONs/ADONs, Treatment Nurses, PT/OTs, Administrators, Dietitians and others responsible for preventing and caring for wound and skin issues for residents.

Objectives:

  1. Describe the physiology of wound healing;
  2. Discuss wound chronicity;
  3. Identify skin anatomy and other tissues and structures often encountered in chronic wounds;
  4. Recognize the most common chronic wound etiologies including pressure injuries/ulcers;
  5. Discuss pressure ulcer prevention strategies and risk assessment methodologies;
  6. List components of the wound assessment (e.g. pressure injury/ulcer staging, measuring) and mandated documentation and reporting requirements;
  7. Describe current treatment interventions for chronic wounds including: debridement, topical therapies, and biophysical agents (e.g. NPWT);
  8. Describe benefits and challenges of using outside wound practitioners and wound clinics for facility patients and residents with chronic wounds;
  9. Identify resources to assist in framing your wound prevention and management program.
